Terminology

HMO — In-network only except emergencies, usually centered around a primary care physician.

PPO — Usually costs more but gives more provider flexibility and typically no referral requirement for specialists.

Deductible — What you pay before the plan starts sharing more of the cost.

Copay — A fixed dollar amount for a service like a doctor visit or prescription.

Out-of-pocket max — The most you pay in-network during the plan year for covered medical expenses.
